Modified sports (grade 7/8) - September 9

Mahopac Central School District Athletics has partnered with a new interscholastic athletic registration platform called FinalForms. Registration for Fall2024 Interscholastic sports will begin on July 26th for High School Sports and August 9 for Modified sports - Grades 7 and 8.

As of 8/1/2023 All physical exams must now be on the Required New York State Health Examination Form.

Please note that "Family Cardiac History Reviewed" must be noted on the physical.

Sports Participation - The Required NYS Health Exam Form needs to be dated within 12 months of the start date of each sport season.
